Residential Sidewalk Complaint Waiver and Agreement. 

Property owners receiving notice of defective sidewalk panel(s) are required by Davenport City Code 12.12 to correct the defect. 

Information regarding your options to remedy the situation were provided in the notice received. This Waiver and Agreement is the instrument property owners must use to identify how they will address their defective sidewalk issue.

Having received a notice that additional defective sidewalk panel(s) were identified at your location, but not included in the estimate you received, you have the option to enroll in the 50/50 cost-share program for the sidewalk panel(s) subject of complaint only, or to enroll in the program for all defective sidewalk panel(s) found at the location, or to make the repairs yourself. 

Complete and submit the form below to let us know how you would like to proceed. Those participating in the cost share program will be invoiced for 50% of the cost of repairs once resurfacing/reconstruction is complete. Property owners may pay the bill within 30 days or have the cost assessed to the property, allowing property owners to spread the cost of resurfacing/reconstruction over time.

After submitting your response, you will receive an email confirmation from "Sent on behalf of the City of Davenport" that includes information on next steps.

I want to participate in the City of Davenport's Residential Sidewalk Cost Share Program. I understand I have received an estimate for repairs and that I will be responsible for 50% of the actual costs, not-to-exceed the estimate, once the work is complete. I also understand I may pay for the balance in full at the time an invoice is received or I may opt to participate in the special assessment process and spread the payments out over time as detailed in the notice I received.
