Sidewalk Cost Share Program Waiver & Agreement

 Step 1 of 1

Residential Sidewalk Cost Share Program Waiver and Agreement Site Under Construction. Do not use.

Property owners receiving notice of a non-compliant sidewalk panel(s) are required by Davenport City Code 12.12 to correct defective/hazardous sidewalks.

Information regarding your options to remedy the situation were provided in the notice received. This Waiver and Agreement is the instrument property owners must use to identify how they will address their hazardous sidewalk issue.

Sidewalks left unrepaired/reconstructed may be condemned by the City, repaired or reconstructed by the City, and 50% of the costs invoiced to the property owner. Non-payment will result in an assessment to the property.

Defective Sidewalk Notice

I have read and understood the Defective Sidewalk Notice information I received.*

Property Owner Election to Correct Defective Sidewalk. Select one option. Defective sidewalk panel(s) must be addressed regardless of whether you participate in the cost share program or not.

I want to participate in the City of Davenport's Residential Sidewalk Cost Share Program. I understand I have received an estimate for repairs and that I will be responsible for 50% of the actual costs, not-to-exceed the estimate, once the work is complete. I also understand I may pay for the balance in full at the time an invoice is received or I may opt to participate in the special assessment process and spread the payments out over time as detailed in the notice I received.
Cost Share Participation: Indicate Agree or Disagree.*
I wish to repair the sidewalk panel(s) on my own and agree I have been provided information on how to proceed with the repairs, permit, and bonding requirements.
